Chosen Hearts is a California-based 501(c)(3) nonprofit organization devoted to supporting individuals and families experiencing homelessness and housing instability. Many of those we serve face complex challenges, including mental health conditions, intellectual disabilities, substance use, and justice system involvement.
Our approach goes beyond addressing immediate needs. We engage through outreach and provide direct services, personalized case management, and ILS (independent living skills) tailored to each individual's unique circumstances.
Our mission is to rebuild lives and restore hope by fostering a sense of community and creating lasting, positive change. Through our hands-on, compassionate work, we empower those we serve to achieve stability, independence, and a secure, fulfilling future.
